Why We Reviewed Extenze

Extenze is one of the most recognised non-prescription male enhancement supplements in the US, made by Biotab Nutraceuticals and sold through retail and online since the early 2000s. Daily softgel, one per day, marketed for erection quality and libido.

It is also the most controversial product in this category. The formula combines yohimbe (a bark extract with documented cardiovascular risk) with DHEA (a hormone precursor banned in competitive sport), most of it inside a proprietary blend that does not disclose individual doses.

The brand has a regulatory history including a 2010 class-action settlement and a 2011 counterfeit incident. Anyone considering Extenze should understand the formula and the risk profile before deciding.

The Active Formula

Three vitamins and minerals are disclosed by dose. The rest sits inside a proprietary blend totalling roughly 358 mg.

Disclosed ingredients. Folate (folic acid) ~500 mcg, Zinc (as oxide) ~25 mg, Niacin ~23 mg. These are the foundational nutrients with the cleanest evidence in the formula, but only folate and zinc have any meaningful research connection to erectile function, and only in men who are deficient in those nutrients to begin with.

Proprietary blend (~358 mg total). The blend contains 22 to 26 ingredients depending on the version, with no per-ingredient dose disclosure. Headline ingredients include:

• Yohimbe bark extract. Active compound yohimbine. Has some evidence for psychogenic ED, but research doses of yohimbine run 5 to 100 mg per day, and there is no way to know how much yohimbine is actually in Extenze.
• DHEA (micronized) and Pregnenolone. Hormone precursors that the body can convert to testosterone and other steroid hormones.
• Horny goat weed (Epimedium). Contains icariin, a natural PDE5 inhibitor. Successful human trials used 526 mg of icariin alone, more than the total weight of the entire Extenze proprietary blend.
• L-Arginine HCl, Korean ginseng, Maca, Tribulus, Muira puama, Damiana, Cnidium, Eleutherococcus, Ginger, Black pepper, GABA, plus another dozen herbs and aminos.

The proprietary blend problem. When 22+ ingredients share 358 mg, individual doses are necessarily small. Innerbody's analysis notes that since the entire blend is smaller than the dose used in single-ingredient ED studies, there is unlikely to be enough of any one ingredient to match research-level effects, while also making it hard to declare the product unsafe based on dose alone.

Clinical Evidence: What the Research Shows

No finished-product trial. No published clinical trial evaluates Extenze as a finished formula. Per Medical News Today, while several individual ingredients have research behind them (yohimbine, ginseng, L-arginine, folate, zinc), there are no studies on the product itself.

Yohimbine evidence is mixed. A 2021 meta-analysis suggested yohimbine may help men with mild psychogenic ED at doses of 5 to 100 mg daily. The NCCIH position is more cautious: not enough evidence to draw conclusions, and yohimbine is associated with cardiac arrhythmia, blood pressure problems, heart attacks, and seizures. The amount of yohimbine in Extenze is not disclosed.

DHEA can raise testosterone. A 2020 meta-analysis confirmed DHEA supplementation can increase testosterone levels. This is also why DHEA is on the WADA prohibited list and is banned by the Olympics, NCAA, NFL, and NBA.

Horny goat weed dose gap. Icariin has demonstrated PDE5 activity, but successful human studies used isolated icariin at 526 mg or more. The total Extenze proprietary blend is 358 mg, with horny goat weed as one of more than twenty components.

Bottom line. Extenze uses ingredients that individually have some research support, but at doses that cannot be matched to research and inside a blend that has never been tested as a finished product.

Who Extenze Is For

Extenze may suit men who want the most widely available non-prescription option and accept the trade-offs:

• Comfortable with a proprietary blend that does not disclose individual ingredient doses
• No history of cardiovascular conditions, hypertension, anxiety, or related contraindications
• Not subject to athletic drug testing
• Willing to commit to consistent daily use over weeks rather than expecting on-demand effects

Who Extenze Is NOT For

This is the longer list, and it matters more than usual for this product.

Men with cardiovascular conditions. Yohimbe raises heart rate and blood pressure. A single dose can raise heart rate by 10 to 20 beats per minute in healthy men. For men with hypertension, heart disease, or arrhythmia, this is not a tolerable side effect, it is a serious risk.

Men on prescription medication. Yohimbe interacts with MAOIs, antidepressants, blood pressure medication, and stimulant medication. The yohimbe component alone is enough reason to clear this product with a physician before starting if you take any prescription drug.

Men on nitrate medication. Counterfeit Extenze has been found contaminated with sildenafil and tadalafil. Even genuine product, taken alongside nitrates, carries enough cardiovascular load to warrant medical sign-off first.

Athletes subject to drug testing. DHEA is on the WADA prohibited list and banned by the Olympics, NCAA, NFL, and NBA. Olympic 400 m gold medallist LaShawn Merritt was suspended in 2010 after taking Extenze.

Men with anxiety disorders, sleep difficulties, or liver conditions. Yohimbe is a stimulant that can trigger or worsen anxiety, insomnia, and palpitations, and is contraindicated in liver disease.

Men under 21. The label restricts use to adult men 21 and over.

What to Expect: Realistic Timeline

Extenze is daily-use, not on-demand, despite some marketing language to the contrary. Yohimbine itself can produce stimulant effects within hours, but the combined herbal effects on libido and erection quality, where they exist, build over weeks of consistent use.

• Days 1 to 7. Stimulant effects from yohimbe possible: increased alertness, faster heart rate, sometimes anxiety or sleep disruption. If these are pronounced, stop and consult a physician.
• Weeks 2 to 4. Subjective changes in libido or arousal may appear in some men, driven primarily by yohimbe and DHEA mechanisms.
• Weeks 4 to 8. The realistic assessment window. If consistent daily use through 8 weeks produces no perceived change, the formula is unlikely to work for this individual.
